Impaired awareness of hypoglycemia (IAH) affects around 25% of adults with type 1 diabetes (T1D). Individuals with IAH exhibit blunted sympathetic counter-regulatory responses (CRR) to hypoglycemia and fail to detect and self-treat hypoglycemia. Technologies such as continuous glucose monitoring (CGM) and hybrid closed-loop pumps (HCL) may reduce both IAH and severe hypoglycemia but the ability of technology to reverse impaired CRR remains unclear. We propose to study the effect of different behavioural change interventions on restoring hypoglycemia awareness and the CRR in adults with type 1 diabetes and IAH during an experimental hypoglycemia clamp using a Sequential Multiple Assignment Randomized Trial (SMART) design in which non-responders at 12 months will be assigned to a second treatment for the next 12 months.
