Half of us will be diagnosed with cancer in our lifetime. While cancer survival has doubled in the last 50 years, longer survivorship brings increased risk of serious secondary morbidity.Cancer survivors have a 3-fold heightened risk of cardiovascular disease, and new evidence is urgently needed to guide optimisation of their cardiovascular health.I will leverage three unique multimodal big data resources (UK Biobank, Oxford Risk Factors and Non- invasive Imaging Study, and Clinical Practice Research Datalink) to: 1. Examine long-term cardiovascular disease risk related to cancer and its therapies in survivors of 20 cancer types across population-based and patient cohorts.2. Evaluate organ-level cardiovascular alterations in cancer survivors using the world’s largest cardiovascular magnetic resonance and cardiac computed tomography imaging databanks, defining granular cardiovascular phenotypic changes related to cancer exposures and new prognostic imaging biomarkers.3. Develop and validate novel cardiovascular risk scores optimised for cancer survivors in large nationally representative samples, for early diagnosis and improved prevention of major cardiovascular diseases.4. Support clinical translation of my risk scores through health economics evaluations, pilot testing, and targeted stakeholder engagement.This proposal will generate new transdisciplinary understanding and deliver transformative innovations in cardio-oncology clinical care.
